Each resident of the St. Nicholas ICF/MR will have an individual service plan developed by an interdisciplinary team tailored to the specific goals and needs of each resident. The interdisciplinary team generally consists of the individual receiving services, a family member/guardian of the resident, a nurse, the case worker (QMRP), and any other individuals that may be helpful in developing and monitoring the service plan.
The services delivered through St. Nicholas’ ICF/MR include:
Residential Services: The group home will be staffed 24 hours a day/7 days a week while any residents are physically at the group home. Staff will assist each resident with meal preparation, bathing/hygiene, medication administration, and other items as needed by each individual resident. Each resident will have his or her private bedroom.
Day Habilitation/Active Treatment: The interdisciplinary team will establish measurable goals and objectives for each resident. One purpose of day habilitation is to work with each individual on life skills, education, job training, and any other area of developmental needs. This daily service, which generally is delivered Monday through Friday during traditional work/school hours, can take place at a third-party day habilitation facility (ex. My Possibilities), community-based employment, traditional schools, and through community-based recreation.
Medical and Therapeutic:
The staff of St. Nicholas will ensure that each resident has the appropriate medical and therapeutic services available to include:
- Medical and Nursing
- Vision
- Dental
- Psychiatric
- Psychology
- Occupational Therapy
- Physical Therapy
- Speech Therapy
- Nutrition
- Any other therapy that would benefit the resident
Recreation:
Community-based recreational outings are a significant part of the St. Nicholas program. Activities include dining out, going to the movies, bowling, going to the park, attending sporting events and fairs, etc. These recreational outings take place periodically throughout the week and weekend.
